Single Parent Discount

If you are a SINGLE parent with a child on your W&L health insurance policy, complete the Single Parent Discount form which entitles you to earn the Live Well W&L discount at the highest rate ($75 per month). In order to qualify, you must be able to answer the following affirmatively:

• I have completed the tokens required to earn the Live Well W&L discount on my health insurance premium for this coming year - OR - I am already benefitting from the discount;
• I am single, legally divorced, or widowed;
• I do not have a Declaration of Domestic Partnership on file with W&L; AND
• I have a child/children on my W&L health insurance plan.

This discount may be renewed annually.
