Upload Covid-19 Vaccination Card

**IMPORTANT**  Before you can upload your vaccination card, you must first add your vaccine date(s) to the application. Click here for instructions.

1. Log into the Daily Attestation App (https://attest.wlu.edu) and click "Report Vaccination". Next click "Upload Card".

2. Click "Choose File" and locate the file on your device to upload. Be sure to click "Save" to complete this step.

3. Your upload will be complete when the Vaccination Card column contains the date and time of upload.
